Blood-stained note is a document in The Sinking City.
It can be found in the Warehouse located at Oakmont Port, Grimhaven Bay, between Piers 2 and 3 on the ground, in a torn off pocket, at the top of the stairs leading to the basement.
It is presumably written by Peter, the bartender at the "Under the Keel" Bar to Lewis Flynn
